Additional Appliance Request Form: AN CLACHAN & JOHN ORR TOWER - Queen’s University generally provides only a fridge and stove in each unit. Per the Lease Agreement, tenant’s require permission to bring in additional appliances. This form is to be completed and submitted to Community Housing by AN CLACHAN or JOHN ORR TOWER tenants who are requesting permissionto bring in additional appliances.
Additional Appliance Request Form: CORE Property – Queen’s University generally provides only a fridge and stove in each unit. Per the Lease Agreement, tenant’s require permission to bring in additional appliances. This form is to be completed and submitted to Community Housing by CORE tenants who are requesting permission to bring in additional appliances.
Authorization Form – This form is to be used to give someone authorization to act on your behalf for the purpose of conducting transactions related to your lease, sub-lease, or assignment agreement (e.g. signing for you, showing your unit, etc.)
Pre Authorized Debit Agreement Form – This form is to be used by tenants who would like to have their rent automatically withdrawn from their bank account on the first banking day of each month. All tenants of a unit must submit pre-authorized payment agreements where the tenants of that unit wish to “split” the monthly rent payment. This form must be re-submitted for each lease period.
Request Permission to Assign or Sublease Unit – This form is to be used by current Community Housing tenants to request permission to assign or sublease their unit to other students.
